Holday Boarding Form – This form is required for boarding during the upcoming holiday periods, including New Year’s, Easter, Memorial Day, the 4th of July, Labor Day, Thanksgiving, and Christmas.
We require the following shots for the pets:
Dogs
- Bordatella (Every 6 months)
- Distemper
- Rabies
- Canine Influenza
Cats
- Feline Leukemia
- Rabies
